Abstract: A 8-year-old pregnant non-descript Red Khandari cow in its fifth parity was presented to Department of Veterinary Gynaecology and Obstetrics, College of Veterinary Science, Parbhani, MAFSU with prolonged dystocia due to twin fetuses. Clinical examination revealed one foetus in an anterior presentation with left lateral head deviation and the other in a posterior presentation. After an attempt to manual extraction which proved futile, a Caesarean section was performed under local anaesthesia using a paramedian approach. The uterus was incised to deliver dead monozygotic twins, followed by uterine lavage and closure using a Lembert followed by Cushing suture pattern. Postoperative care included antibiotics, anti-inflammatory drugs, IV fluids, and wound management, leading to smooth recovery.
