Successful treatment of multi-drug resistant Klebsiella pneumoniae induced pyometra in a bitch with cefoperazone-sulbactam (Valocef®, Zuevets) and enzyme therapy

Abstract: This case report highlights the effective treatment of pyometra in a 5-year-old Labrador Retriever, caused by Klebsiella pneumoniae demonstrating multi-drug resistance (MDR). Antibiotic susceptibility testing revealed resistance to several commonly used antibiotics, including amoxicillin-clavulanate, gentamicin, and ciprofloxacin, but showed sensitivity to cefoperazone/sulbactam, a third-generation cephalosporin paired with a beta-lactamase inhibitor. The therapeutic approach combined cefoperazone/sulbactam with trypsin-chymotrypsin, an enzyme duo used to improve drug efficacy by promoting the clearance of necrotic tissue and enhancing inflammatory resolution. This treatment strategy resulted in the complete recovery of the dog, emphasizing the potential of this combination in managing severe open pyometra cases where surgery is not a viable option.
