Transpelvic urethrostomy: An alternative approach to unsuccessful perineal urethrostomy in a tom cat

Abstract: A five-year-old Persian Tom cat was presented to Small Animal Surgery Outpatient Unit of Madras Veterinary College Teaching Hospital with a history of dysuria, stranguria and recurrent urinary tract infection for the past six months and had already undergone perineal urethrostomy procedure for urethral obstruction two months prior. Clinical examination revealed distended abdomen, which on palpation revealed an overtly distended bladder. Lateral abdominal radiograph revealed distended bladder. On abdominal ultrasonography, an irregular, mildly hyperechoic and thickened urinary bladder wall was found. Hematobiochemical values revealed neutrophilia and slightly elevated levels of blood urea nitrogen and creatinine. Catheterization was unsuccessful due to stenosis of urethral opening. Cystocentesis was performed and around 30ml of blood-stained urine was relieved. After stabilization of the animal with fluid therapy, opioids and antibiotics, trans-pelvic urethrostomy was performed under general anaesthesia and urethral obstruction was relieved. Urethral patency was maintained with appropriate post-operative care and follow up treatments. The animal had an uneventful recovery.
